How to create a fillable PDF from a Word document

Lay out the form in Word

Type the labels and leave a clear box or underline where each answer goes. Use a table for aligned fields. Skip the Developer tab content controls, because no converter keeps them.
Convert to PDF

Open the .docx in Word to PDF, or use File, Export in Word. Check that the layout is right, because the fields will sit on top of these pages.
Add the fields

Open the PDF in Create fillable PDF. Draw a text field over each box, a checkbox over each tick square, and name every field.
Test it

Download the form, open it in a PDF reader, fill two fields and save. Reopen the saved copy and confirm the values are still there.

What happens to Word form fields on conversion

Word has two kinds of form control: legacy form fields and content controls from the Developer tab. Both work inside Word when the document is protected for filling in forms. Neither one is a PDF form field, and no exporter has a translation for them. Word's own export shows the grey box as a drawing and stops there. The browser converter draws the text and the box borders and counts the control as a dropped object on its receipt, so at least you know.

That surprises people because Acrobat can sometimes detect those boxes and turn them into fields afterwards. The detection is a guess based on rectangles and underlines, and it misses as often as it hits. Adding the fields deliberately takes less time than correcting a guess.

Design the form in Word for the fields that come later

A PDF field is a rectangle drawn over the page, so the page needs a place for it. Give each answer a box drawn with table borders or a row of underscores, and leave enough height for a typed line. Keep labels to the left or above their box, and keep boxes in a grid, because aligned fields are quicker to draw and easier to tab through.

Do not use text boxes floating over the page for labels. They convert fine, but they move if the document is edited later and the fields on the PDF will not follow. Plain paragraphs and tables stay put.

How to make a Word document fillable, and why it stops at Word

You cannot make a fillable PDF in Word alone, if fillable means fillable in a PDF reader. Word can make a form that is fillable inside Word, and you can send the .docx with the form protection on. That works when every recipient has Word and nobody needs to sign or print. The moment someone opens it on a phone or in a browser, the protection and the controls behave differently or vanish.

A PDF form fills the same way in every reader, on every device, and stays readable for years. That is why the PDF is the deliverable and Word is the layout tool.

Fields are anchored to page coordinates, not to the text under them. Edit the Word document, convert again, and the fields drawn on the old PDF no longer line up. For a small change, add the fields again; it takes a few minutes for a two-page form. For a form that changes often, freeze the layout first and change the wording last.
