Choose a field type, then drag over the place where an answer belongs. Move a field by dragging it.

Name each field
Give each field a useful name, mark required answers and add the choices for dropdown fields.

Download the checked form
The output is reopened. Every new field, page and original selectable character must pass before download.
What this tool does not create
This tool creates text, checkbox, date and dropdown fields. It does not create certificate-backed digital signature fields, calculation scripts, submit buttons or conditional form logic. Sign PDF can add a visible signature after the form is filled.

Questions about creating PDF forms
- What fields can I add?
- You can add single-line text, long text, checkboxes, date text fields and dropdown menus. Existing fields stay in the PDF.
- Will the fields remain editable?
- Yes. The download contains interactive AcroForm fields. It does not flatten or rasterize the PDF.
- Can I fill the form after creating it?
- Yes. Open the result in a PDF reader or use the pdfpix Fill PDF form tool.
- Is my PDF uploaded?
- No. The PDF, field names and dropdown choices stay in this browser tab. Analytics receive only aggregate counts.
- How is the form checked?
- The tool reopens the output and checks every new field name, type, page widget and required setting. It also verifies every original page and selectable character.
- Can I add a signature field or a submit button?
- No. This tool creates text, checkbox, date and dropdown fields only. Certificate-backed signature fields, calculation scripts, submit buttons and conditional logic are out of scope. Sign PDF can place a visible signature after the form is filled.
